The best way we provide for submitting Your claim:

• Provide the copies of all documents of your claim - a detailed description of the required documents can be found in the information below.
• Write the following text on the receipt/invoice/document: Presented to …. (current date) on a claim to Bulgaria Insurance AD.   
• Fill out and submit a DECLARATION to Claim for reimbursement of expenses up to 500 (five hundred) BGN, with the documents of your claim.
• If you don't have a registration yet, you can quickly and easily create an account using the Register button.

IMPORTANT NOTE: We would like to draw the attention of our customers and partners to be aware of some technical details required by the Claims Department, that will optimize the claims handling process, as follows:

• Sending of legible copies of the documents
• When copying the documents, do not obscure data from the invoice/receipt. Documents should be submitted individually.   

Claims of up to BGN 500.00 will be proceeded without presentation of original financial documents, with filed and applied DECLARATION to Claim for reimbursement of expenses up to 500 (five hundred) BGN, therefore the Insurer reserves the right to request the original financial documents for review during the claim assessment process.

Documents required by the Insurer for filing a claim upon occurrence of an insurance event 

When examined by a specialist:   

• Claim form for reimbursement of expenses - not necessary when claim is online
• Medical report or outpatient card certifying an examination by a specialist, which must contain date, seal, UIN of the doctor, the names of the insured person, medical history, objective condition, made diagnosis and prescribed tests and therapy
• Receipt 
• Results from examinations (laboratory, X-ray, etc.)   
• Invoices with cash receipts for incurred medical expenses in the name of the insured person - the individual prices of health services and goods must be described and the prices must be separated by items

After hospital treatment:

• Claim form for reimbursement of expenses - not necessary when claim is online
• Epicrisis/ medical report for hospital stay
• Application for selection of a medical team   
• Stickers of consumables used in operations (if paid by the insured person)
• Invoices with cash receipts for incurred medical expenses in the name of the insured person - the individual prices of health services and goods must be described and the prices must be separated by items   

* In case of prescribed additional therapy, after hospital treatment the prescription should be attached.

Reimbursement of expenses of prescribed vision correction glasses or corrective lenses:

• Claim form for reimbursement of expenses - not necessary when claim is online
• Medical report or outpatient card certifying an examination by an ophthalmologist performed in a medical facility, which must contain date, seal, UIN of the doctor, the names of the insured person, made diagnosis for eye visus change of not less than 0.5 diopters
• Prescription for the vision correction
• Stickers/ packages of the prescribed dioptric glasses/ lenses
• Invoices with cash receipts for incurred medical expenses in the name of the insured person - the individual prices of health services and goods must be described and the prices must be separated by items

Reimbursement of expenses under Dental Care package:

• Claim form for reimbursement of expenses - not necessary when claim is online
• Medical report or outpatient card certifying an examination by a specialist, which must contain date, seal, UIN of the doctor, the names of the insured person, treatment description, dental status, made diagnosis, prescribed tests and therapy
• Sectoral X-ray for treatment of pulpitis and periodontitis
• Invoices with cash receipts for incurred medical expenses in the name of the insured person - the individual prices of health services and goods must be described and the prices must be separated by items   

* In case that you are filing a claim for dental treatment for first time you should attach panoramic X-ray taken before treatment.

After physiotherapy procedures:

• Claim form for reimbursement of expenses - not necessary when claim is online
• Medical report or outpatient card certifying an examination by a specialist (Neurologist/ Оrthopedist), which must contain date, seal, UIN of the doctor, the names of the insured person, medical history, objective condition, made diagnosis and prescribed physiotherapy procedures
• Procedure card for performed physiotherapy procedures
• Invoices with cash receipts for incurred medical expenses in the name of the insured person - the individual prices of health services and goods must be described and the prices must be separated by items
